Description

Authorize to enter into an amendment to an existing contract with Westport Healthcare Management, Inc. d/b/a Pacific Health Policy Group, Lake Bluff, IL, to develop and implement a comprehensive evaluation of the Removable Prosthodontic Devices (Dentures) benefit for adult nursing home residents, as well as exercising a contract renewal option by extending the completion date from December 30, 2025 to December 30, 2027 to incorporate the 1115 (a) Substance Use Disorder and Severely Mental Illness Waiver amendment extension granted by the Centers for Medicaid and Medicare Services in June 2023, and increasing the price limitation by $731,480 from $1,334,220 to $2,065,700. Effective retroactive to July 1, 2023, upon G&C approval. 50% Federal Funds, 36% General Funds, 14% Other Funds (New Hampshire Granite Advantage Health Care Trust Fund)
