Authorize to enter into a sole source amendment to an existing contract with STCHeatlh, LLC, Scottsdale, AZ (originally approved by G&C on 10/21/15, Item #11), for maintenance and support for the New Hampshire Electronic Disease Surveillance System, by exercising a contract renewal option, by increasing the price limitation by $316,056 from $3,251,460 to $3,567,516 and by extending the completion date from June 30, 2023 to June 30, 2024.
