Description
Authorize to enter into a sole source amendment to an existing grant agreement with Community Health Access Network, Newmarket, NH (originally approved by G&C on 10/13/21, Item #25), to improve asthma-related services in both medical and community organizations to reduce asthma-related illness and deaths, by increasing the price limitation by $108,500 from $360,366 to $468,866, with no change to the contract completion date of August 31, 2024. Effective July 1, 2023, upon G&C approval, whichever is later. 100% Federal Funds
