Authorize to amend an existing contract with Granite Young Men’s Christian Association (YMCA), Manchester, NH, to increase the number of healthcare providers referring individuals with or at risk for chronic conditions such as diabetes, hypertension, and arthritis to YMCA programs, with the aim of improving health outcomes, by exercising a renewal option by increasing the price limitation by $340,000 from $1,034,000 to $1,374,000 and extending the completion date from June 30, 2023 to June 30, 2025.
