Description
Authorize to enter into a sole source amendment to an existing contract with the County of Cheshire, Keene, NH (originally approved by G&C on 9/23/20, Item #14), to continue providing Care Management Entity services and Enhanced Care Coordination through the Families and Systems Together Forward program, by exercising a contract renewal option, by increasing the price limitation by $3,000,000 from $4,275,000 to $7,275,000 and by extending the completion date from June 30, 2023 to June 30, 2025. Effective upon G&C approval. 10% Federal Funds, 90% General Funds
