Description

Authorize to enter into a sole source agreement with HealthFirst Family Care Center, Inc., Franklin, NH, in the amount of $775,750 to establish and operate a Federally Qualified Heath Center at the current Mascoma Community Health Center location in Canaan, NH. Effective July 1, 2023, upon G&C approval through the date the Contractor has fulfilled all obligations as specified in Exhibit C, Payment Terms as detailed in the letter dated March 21, 2023. 100% General Funds
