Authorize to enter into a sole source amendment to an existing contract with Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 5/19/21, Item #23), to add funding and expand the scope of work to support training and technical assistance for the thirteen New Hampshire Small Rural Hospitals, by increasing the price limitation by $1,058,657 from $5,662,818 to $6,721,475 with no change to the contract completion date of May 31, 2024. Effective upon G&C approval. 100% Federal Funds.
