Authorize to enter into a sole source amendment to an existing contract with Evident Change, Madison, WI, to continue to review and revise the Structured Decision Making assessment tools used by the Bureau of Elderly and Adult Services serving individuals in the Adult Protective Services program, by extending a contract renewal option with no change to the price limitation of $269,300 and extending the completion date from June 30, 2023 to June 30, 2024.
