Description

Authorize to amend the existing provider agreement template for the Breast & Cervical Cancer Program (originally approved by G&C on 11-14-18, item #20), to include additional optional services for cardiovascular screening and to change the program name to the New Hampshire Health Lives, by increasing the price by $1,919,080 from $1,838,160 to $3,757,240 and by extending the completion date from June 30, 2022 to June 30, 2024. 63% Federal, 37% General Funds.
