Authorize to enter into agreements with the vendors as detailed in letter dated May 20, 2014, for Wraparound Facilitator/Care Coordinator services that will provide family centered planning and facilitation along with planned development within the System of Care for children, youth and families experiencing difficulties due to the child or youth’s severe emotions disturbance, in an amount not to exceed $414,000. Effective July 1, 2014 or upon G&C approval, whichever is later, through June 30, 2016. 100% Federal Funds.
