Authorize to enter into agreement with vendors as detailed in letter dated May 27, 2014, to provide In Home Care, In Home Health Aide Level of Care, In Home Nursing Level of Care Services, and Adult Day Program Services to support elderly and disabled adults in order for them to remain in their homes and communities, in an amount not to exceed $7,319,907 in the aggregate. Effective July 1, 2014 or upon G&C approval, whichever is later, through June 30, 2015. 47% Federal, 53% General Funds.
