Description
Authorize to amend a sole source agreement with Area Agency of Greater Nashua Inc., d/b/a Gateways Community Services, Nashua, NH (originally approved by G&C on 12-8-10, item #140), to provide Financial Management Services for support and respite services for caregivers, by reinstating the Alzheimer’s Disease and Related Disorders program funding, by adding dollars to the Family Caregiver Program, and by increasing the price by $295,000 from $1,534,803.01 to $1,829,803.01. Effective upon G&C approval through December 31, 2013. 51% Federal, 49% General Funds.
