Description
Authorize the Bureau of Population Health and Community Services, Maternal and Child Health Section to amend a sole source agreement with Child Health Services, Manchester, NH, (originally approved by G&C on 6-20-12, item #115), to provide adolescent health services, by increasing the price by $50,000 from $374,616 to $424,616. Effective July 1, 2013 or upon G&C approval, whichever is later. 10% Federal, 90% General Funds.
