Description
Authorize the Bureau of Population Health and Community Services, Healthy Eating and Physical Activity Section, Obesity Prevention Program, to enter into an agreement with JSI Research and Training Institute Inc., d/b/a Community Health Institute, Bow, NH, to provide Nutrition and Physical Activity Self-Assessment for Child Care Services for no fewer than six child care programs, in an amount not to exceed $15,521. Effective upon G&C approval through June 30, 2012. 100% Federal Funds.
