Authorize to enter into a sole source amendment to an existing contract with Dr. David A. Bogacz, Concord, NH (originally approved by G&C on 5/19/21, Item #17), to continue providing dental services to the residents of Glencliff Home, by exercising a contract renewal option, by increasing the price limitation by $43,200 from $43,200 to $86,400 and extending the completion date from June 30, 2023 to June 30, 2025.
