Description

Authorize to enter into a sole source amendment to an existing contract with Fivebase, LLC, Hampton, NJ (originally approved by G&C on 8/18/21, Item #36), to continue providing information technology support for the New Hampshire Hospital electronic health record system, MyAvatar, by exercising a contract renewal option, by increasing the price limitation by $300,000 from $632,500 to $932,500 and extending the completion date from August 31, 2023 to August 31, 2024. Effective September 1, 2023, upon G&C approval. 100% Other Funds (Inter-Agency)
